Current demo behavior
The form lets you prepare and preview a service request in your browser. JavaScript prevents submission. The values remain in the open page and are cleared when the page is refreshed or closed.
Selecting the phone link opens your device or calling service. Any information shared during a real call is handled outside this website concept.
Information shown in the form
The preview can contain your name, mobile number, vehicle details, VIN or chassis number, preferred date, and description of the concern. These fields are suitable for a service-intake request, but production collection must remain limited to what the workshop actually needs.

Browser storage and hosting
The site stores one preference named eaw-concept-theme in local browser storage so the selected light or dark theme can remain on the device. It is not used for advertising, profiling, or service-request data.
This site code does not set cookies. A consent banner is therefore unnecessary for the current concept. If analytics, advertising tools, embedded social media, or other optional tracking are added later, the notice and consent controls must be reassessed before launch.
When the files are served online, the hosting provider may receive ordinary technical request data such as an IP address and browser headers. The production owner must confirm the hosting provider, processing purpose, retention period, security controls, and any cross-border transfer before publishing a final notice.
Before a real form goes live
A production privacy notice must match the workshop's actual practices. Philippine privacy guidance calls for clear information about the data collected, purpose and basis, processing method, recipients, controller identity, retention, rights, risks, and complaint route.
Confirm the registered business name, physical address, privacy contact, hosting and form providers, storage location, retention schedule, disposal method, security controls, and third-party recipients before enabling submission.

When personal data is processed, the Data Privacy Act of 2012 provides rights that include being informed, access, objection, rectification, erasure or blocking, data portability, damages, and filing a complaint.
